Abstract: Introduction: The musculoskeletal system relies on the interaction between various tissue structures, such as muscles and bones, to efficiently transmit forces and maintain joint stability. The myotendinous junction (MTJ) is a critical point where muscle and tendon meet for this force transmission; however, it is also a vulnerable area prone to injuries. Injuries to the calcaneal tendon can significantly disrupt these complex interactions, requiring recovery processes that involve cellular inflammation and tissue remodelling. In this context, studying protein patterns in affected tissues, such as the gastrocnemius muscle and the MTJ, provides insights into molecular changes during the recovery process. Objectives: The aim of this study was to investigate the protein profile in the gastrocnemius muscle and the MTJ after a partial lesion of the calcaneal tendon to identify proteins with differential expression and biological processes present during the recovery stages. Methods: Thirty Wistar rats were divided into five experimental groups: a control group and four groups subjected to partial tendon lesions, with analyses conducted at different post-injury times: 3, 14, 28, and 55 days. The lesion was performed, followed by the collection of gastrocnemius muscles and MTJs for proteomic analysis. Proteins extracted from the muscles were quantified and analysed using LC-MS/MS mass spectrometry and subsequent bioinformatic analysis to identify regulated proteins and enriched biological pathways. Results: The results revealed significant changes in the proteomic profile over time. Three days post-lesion, there was strong regulation of proteins related to inflammation and oxidative stress, indicating an initial defensive response. By 14 days, the profile shifted to an adaptive phase, focusing on energy metabolism and structural reorganisation. At 28 days, proteins associated with regeneration and inflammation control were prominent. Finally, at 55 days, the profile suggested a return to homeostasis, indicating that the tissue had entered the advanced recovery phase. Conclusion: This study revealed the complexity and temporal dynamics of molecular responses following a partial Achilles tendon lesion. The specific regulation of proteins associated with inflammatory response, energy metabolism, and cellular structure highlights the adaptive role of the gastrocnemius muscle and the MTJ. These findings provide a foundation for developing therapeutic interventions aimed at accelerating functional recovery and preventing future injuries in musculoskeletal structures.
